ENROLLMENT

Before enrollment can be completed for any child, a face to face interview is required,
we also require the following forms along with the registration fee, for each child,
all forms must be filled out in it’s entirety, signed, and dated.

* Child Enrollment Record
* Medical Emergency Statement Form with copy of insurance card (must be notarized)
* Updated Immunization Record
* Parent Handbook Acknowledgement Form (must be notarized)
* Parent/Provider Contract (must be notarized)

Along with the listed forms, parents are also required to have the
following personal items available on the child’s first day of attendance.

Mandatory
* Toothbrush/Toothpaste
* Slippers
* Blanket
* Change of Clothes (seasonal)
* Diapers/Training Pants (refilled monthly)
* Diaper Wipes (refilled monthly)
* Sippy Cups/Ready-to-Feed Bottles (daily)

Optional
* Kleenex
* Hand Soap
* Paper Towel
* Clorox Wipes

REGISTRATION & TUITION

One Time Registration fee of $50.00

TUITION

1-2 days drop ins: $35.00
1-5 days full time: $100.00



HOURS OF OPERATION

Here at Earth’s Angels Home Daycare we try to provide 100% quality childcare by conveniently keeping our doors open to our parents. We do however, reserve the right to close due to most federal holidays, weather, personal emergencies, and/or vacation every year. Please understand that you, the parents, are responsible for finding and paying for backup care for your child.

DAILY HOURS:

Monday through Friday
5:00 am to 5:30 pm
We are closed Saturday and Sunday

DISCPLINE POLICY

Children are explained the House Rules frequently so that everyone knows and remembers the guidelines. Once your child is familiar with the rules, but refuses to abide by them, the following techniques are used:

Reminders, Redirection, Removal of Privileges, age appropriate time out, and as a last resort when the child’s behavior is constantly upsetting or dangerous to myself or others, a conference will be called with the parents.

You may be called to remove your child if his or her behavior prevents me from properly caring for the other children. If your child’s behavioral problems continues, other childcare arrangements will have to be made for the safety and well being of everyone in the Daycare Home.


ILLNESS POLICY

Here at Earth’s Angels, we care about the well being of ALL of our children, we are a small environment which should make it easier to control the spread of infectious diseases, but this must first begin with the parents. If your child is not feeling well for any reason, please do not send him/her to the daycare home. We will administer prescription medication once daily only from it’s original container with the child’s name and dosage. Over the counter medicine will only be given with a parent and /or physicians note detailing dosage and the reason the medication is to be given.

We will only administer medication once per day at 12:00pm with lunch. A medication form has to be completed and signed by the parent for the week of treatment needed. Any child with a fever of 100.3 degrees or higher and any other contagious symptoms is not pe rmitted to stay in the Daycare Home. Any child with a severe “mucus” nose and/or eye will be sent home if the child is too uncomfortable to attend, this includes, but is not limited to constant crying, hard time sleeping or breathing, or loss of appetite.

If your child is sent home with fever, diarrhea, or vomiting, he or she must be free of their symptoms for exactly 24 hours before returning to the daycare home. In the event your child is sent home and has to go to the doctor and receives a prescription medication or over the counter medication, we must have a return to daycare note which specifically states the child is NOT contagious. At Earth’s Angels Childcare, we require all adults that are in contact with your child to be CPR and First Aid certified.

We will gladly administer First Aid to minor injuries and notify parents by phone or email and a completed accident report. In the event of a major illness, we will immediately noti f y 911 and then the parents in which your child will then be transported to Newton General, if necessary (please provide us with a copy of your child’s insurance card).
